With more points of contact than 12-point openings, the spline-shaped box end gets a better grip on nuts and bolts and is less likely to round off corners. Rotate these wrenches without having to remove and reposition them. They're commonly used where you don't have enough space for full turns. To switch between loosening and tightening, flip them over.
